After scheduling your new patient appointment, you will receive intake and consent forms by email. These forms are also available in your Optimantra Patient Portal. The portal is HIPAA-compliant and securely stores:

Upload Recent Medical Records

If you have lab work, imaging, or test results from the past year completed by another provider, please bring them to your appointment or upload them to the Documents section of your Patient Portal in advance.

For in-person visits, wear comfortable clothing to allow for a physical exam if one is needed.

Salus uses Fullscript, an online supplement dispensary, to provide access to high-quality, practitioner-recommended supplements. We are happy to assist you with placing or managing supplement orders during or after your visit.
