Your initial appointment is focused on understanding the full picture of your health. During this visit, we will:
Conduct a thorough review of your medical concerns and current symptoms
Take a comprehensive medical, lifestyle, and health history
Review medications and supplements you are currently taking
Analyze past lab work and determine if additional testing is needed
Perform a physical exam, if appropriate
Clarify your health goals and priorities
Explain our treatment recommendations and outline a personalized action plan
Provide guidance on high-quality supplements and individualized dosing, when recommended
Our goal is to move beyond symptoms and identify the root factors influencing your health.
After scheduling your new patient appointment, you will receive intake and consent forms by email. These forms are also available in your Optimantra Patient Portal. The portal is HIPAA-compliant and securely stores:

Please complete all forms before your appointment to allow us to prepare thoroughly for your visit.

If you have lab work, imaging, or test results from the past year completed by another provider, please bring them to your appointment or upload them to the Documents section of your Patient Portal in advance.

Salus uses Fullscript, an online supplement dispensary, to provide access to high-quality, practitioner-recommended supplements. We are happy to assist you with placing or managing supplement orders during or after your visit.

Please note: We have a 48-hour cancellation policy with a $50 fee to respect the needs of patients on our waiting list.
